Descripción : Human dentition is an important tool to determinate biological variability schemes. Mainly the morphological trait that each dental piece presents, as it is the case of the primary or permanent upper first molars, which presents a structure called Cusp of Carabelli, which is located on the mesial aspect of the mesiopalatal cusp bilateral or in a single quadrant, it also can be observed in 4 categories like Smooth surface, Single pit or furrow, Double furrow or Y-shaped furrow, Slight protuberance or small cusp and Large cusp. The objective is to determine the prevalence of the cusp of Carabelli in permanent upper first molars in patients who attend on the Center of Dental Care of the University of the Americas in Quito. Materials and Methods: An observational, descriptive and transversal study that clinically evaluates the presence of the Carabelli tubercle in the definitive upper first molar, and the morphology that it adopts. This study is carried out by means of models of studies in plaster and photographic record. Results: The prevalence level of this cusp is low since it was only observed in 17 patients in 28.3 percent of 100 percent of participants that presented it, according to gender, in the male gender, 24 percent of the participants presented this morphological trait, with the large cusp being the one with most percentage (12 percent), while in the female gender presented 28.3 percent of the total sample, having a predilection for the cusp Large 10 percent and small cusp or slight protuberance equal 10 percent. Conclusion: there was a minimal difference between females and males in the level of prevalence and in the total sample the prevalence level in Quito is population is low.
