Objective: To determine the prevalence of caries in the first definitive molar in children from 6 to 8 years old who attend the dental clinic of the University of the Americas from October to November 2017. Materials and Methods: The present investigation was carried out through an observational and descriptive study. To carry out this work, the first erupted molars of the children who attended the Odontological Clinic of the University of the Americas were taken into account through the application of the international ICDAS caries detection and diagnosis system. The sample consisted of 57 children. The data collected were organized in a data table in the SPSS22 program, and analyzed using the Chi-square statistical test. Resultados: It was observed that in the first molars there was a predominance of healthy pieces in all ages and genres with ICDAS code of 0, followed by code 2 (white or brown stain in wet enamel) found in piece 36 a 28.1% and in piece 46, 24.6%. It should be noted that in the following codes (3, 4, 5 and 6) were found in very low percentages. Conclusions: all the definitive first molars were analyzed, taking into account that the presence of children was relatively more significant than the girls, it could be observed that even so the prevalence of caries in male patients was higher; It was also found that the most affected first molar is piece 36 and that most often children of 6 years had some type of decay in that piece, this because there is no exfoliation of temporary pieces in the place where it erupts, therefore many times the parents do not recognize that this is a definitive piece; in turn, it is also associated with the number of children present in the family, the mother's attitude regarding oral health and the degree of preparation of the parents.en
