Obesity and dyslipidemia are current health problems in Ecuador. In particular, type 2 diabetes mellitus (T2DM) is associated with high prevalence and mortality rates. This is a chronic disease that results from genetic and environmental factors. Studies suggest that T2DM is associated with chronic inflammation, which include features such as increased circulating proinflammatory cytokines, i.e. IL-6, IL-1β, TNF-α, and fibrosis in pancreatic islets, among others. Additionally, it has been reported the downregulation of miR- 146a and miR-155 in T2DM. MicroRNAs are molecules that target messenger RNA (mRNA) to regulate gene expression. In the present investigation, serum concentration of miR-146a and miR-155 were measured in patients with T2DM, under treatment with metformin (G1) or metformin plus L. mutabilis for 12 weeks. In addition, serum concentrations of miR-146a and miR-155 in participants who didn’t have diabetes were quantified. To achieve this, total RNA was isolated from serum samples and reverse transcription was performed. Quantification was performed by Quantitative PCR (qPCR). A tendency to increase levels of miR-146a in treatment with metformin and L. mutabilis was found, however, no statistically significant variations were found within the treatment group. The Spearman correlation coefficient between miR- 146a and miR-155 was 0.412 (p = 0.030) at baseline and 0.446 (p = 0.017) at the end of the 12 weeks of treatment in patients with DMT2.en
