dc.descriptionDespite screening programs based on conventional cytology implemented in Ecuador, cervical cancer remains a neoplasia with high rates of incidence and mortality among women. Internationally implementation of DNA testing virus (HPV) high risk HPV has proven its profitability and viability, however the costeffective ness of this intervention has not yet been evaluated for the context of Ecuador. The aim of this study was to determine the cost-effectiveness of different screening strategies for cervical cancer in Ecuador from the perspective of a third party payer and society, by adapting the model CERVIVAC. It was compared the use of molecular tests for HPV DNA for high risk genotypes versus using conventional cytology. The diagnostic performance of each strategy was obtained from the review of the published literature. The direct and indirect costs associated with the detection and treatment of cervical cancer in Ecuador using expert interviews and field data collection services in different public and private health Quito, Guayaquil, Cuenca and Loja were estimated. The effectiveness of screening strategies was measured in life years gained, years of disability-adjusted life and incremental cost-effectiveness ratio with an equivalent life expectancy horizon, and discount rates of 3 percent calculated annually for both costs and effects. The robustness of the results was assessed by a deterministic sensitivity analysis. The results suggest that the screening using tests based on the detection of DNA genotypes of high-risk HPV is a highly cost-effective and potentially costthifty strategy in comparison with the cytology that is used today to the context of Ecuador.en
