dc.descriptionClass II patients present significant skeletal imbalance and orthodontic treatment seeks to correct or mask this discrepancy. This clinical case reports an 18 year old female patient with skeletal class II by maxillary protrusion with vertical growth, left molar class I and right II, canine infraocclusion, severe crowding in the maxillary arch and moderate in the mandibular arch, proclination and protrusion of upper and lower incisors, decreased overbit and normal overjet. The objectives of the treatment were to resolve the upper and lower crowding, canine and molar class I, adequate overjet and overbite, obtain good balance and occlusal function. The treatment consisted of extraction of four first premolars and fixed appliances with the Roth technique, aided by sliding jig and class II elastics. After 21 months of treatment, canine and molar class I were obtained, adequate overjet, overbite and optimal occlusion, improving the aesthetics and self esteem of the patient. When a treatment with dental extractions is carried out there are important factors that must be taken into account from the beginning, such as how many and which pieces are to be extracted, the need for anchoring and the space closure mechanism, since if they are handled in a can achieve not only improvements at the occlusal level, but improvements in the profile of the patient and long term stability.en
dc.description.abstractLos pacientes clase II presentan desbalance esquelético significativo y el tratamiento de ortodoncia busca corregir o enmascarar esta discrepancia. Este informe de caso clínico reporta una paciente de sexo femenino de 18 años de edad que presenta clase II esqueletal por protrusión maxilar con crecimiento vertical, clase I molar izquierda y II derecha, infraoclusión de caninos, apiñamiento severo en el arco maxilar y moderado en el arco mandibular, proinclinación y protrusión de incisivos superiores e inferiores, overbite disminuido y overjet normal. Los objetivos del tratamiento fueron resolver el apiñamiento superior e inferior, obtener clase I canina y molar, adecuado overjet y overbite, obtener buen balance y función oclusal. El tratamiento consistió en extracción de cuatro primeros premolares y aparatología fija con la técnica Roth, cierre de espacios con técnica de deslizamiento; se necesitó el uso de un sliding jig y ligas intemaxilares de clase II. Después de 21 meses de tratamiento se obtuvieron clase I canina y molar bilateral, adecuado overjet, overbite y óptima oclusión, mejorando la estética y autoestima de la paciente. Cuando se realiza un tratamiento con extracciones dentales hay factores importantes que se deben tomar en cuenta desde el inicio, como las piezas que se van a extraer, la necesidad de anclaje y el mecanismo de cierre de espacio, ya que si se manejan de forma correcta pueden lograr no solo mejoras a nivel oclusal, sino mejoras en el perfil del paciente y estabilidad a largo plazo.es_ES
