Resumen : Introducción: Los accidentes ofídicos son causados por la mordedura de varias especies de serpientes venenosas. Estas son emergencias que amenazan la vida y son prevalentes en las selvas tropicales. Al momento son consideradas como enfermedades olvidadas por la OMS, por la falta de conocimientos/evidencias de su epidemiología y factores de riesgo. El envenenamiento puede ser leve, moderado y/o severo y de esto depende el tratamiento con el antiveneno y complicaciones. En Ecuador, los accidentes ofídicos son considerados un problema de salud pública por su alta incidencia, morbilidad y mortalidad. Para los años 2017, 2018 y 2019 MSP a nivel nacional reportó 1.448, 1.297 y 1.489 accidentes ofídicos, respectivamente, con la mayoría de casos en provincias de la Costa y Amazonía. En el 2014 en la provincia de Esmeraldas, se reportó alta incidencia, predominando en hombres jóvenes relacionados con actividades agrícolas. Objetivo: Describir la epidemiología, factores de riesgo y clínica de los accidentes ofídicos registrados en el período 2017 a 2019 en el Hospital del Sur “Delfina Torres de Concha” Esmeraldas – Ecuador. Materiales y métodos: Estudio descriptivo de corte transversal a partir del análisis retrospectivo de las historias clínicas de los pacientes atendidos por accidentes ofídicos. Se registró variables sociodemográficas y clínicas como edad, sexo, y cantón de procedencia del paciente, mes de la mordedura, especie de serpiente, síntomas y signos, sitio anatómico de la mordedura, primeros auxilios y complicaciones. Los datos recolectados en Microsoft Excel, se analizaron mediante JASP y SPSS. El análisis estadístico incluyó frecuencias absolutas, proporciones y tasas. Para la exploración de asociaciones entre las variables se emplearon t student y chi cuadrado, considerándose significativa a p menor que 0,05. Resultados: En total se registraron 110 casos, 26 (24 por ciento), 22 (20 por ciento) y 62 (56 por ciento) para los años 2017, 2018 y 2019, respectivamente. Hubo una muerte (0,9 por ciento) en el período de estudio. El sexo masculino con 76 (69,1 por ciento) casos. Los grupos de edad entre 11-20 años con 25 (22,7 por ciento) y de 0-10 años con 21 (19,1 por ciento). Los agricultores con 34 (31 por ciento), estudiantes con 30 (27,3 por ciento). Procedentes del cantón Quinindé 48 (43,por ciento); provenían de zonas rurales 68 (61,8 por ciento); 13 (11,8 por ciento), 12 (10,9 por ciento) y 14 (12,7 por ciento) casos acudieron los meses de mayo, julio y agosto, respectivamente. Bothrops atrox fue la especie de serpiente agresora en 43 (41,7 por ciento) casos. Se diagnosticaron en 38 (36,9 por ciento ) con severidad moderada y severa. Como primeros auxilios recibidos están los sueros antiofídicos en 43 (37,7 por ciento) con la administración de 5 a 8 ampollas en 28 (27,2 por ciento) casos. En hospitalización el 87.3 por ciento recibieron entre 4 a 12 ampollas. El pie derecho fue afectado en 35 (34,3) casos. Entre las manifestaciones clínicas predominaron las locales con edema en 91 (41,2 por ciento) y dolor en 84 (38 por ciento). Las manifestaciones sistémicas frecuentes fueron hipertensión en 33 (17,6 por ciento) y taquicardia en 26 (13,9 por ciento) casos. Complicaciones como síndrome compartimental y necrosis se presentaron en 7 (6,2 por ciento) casos. Conclusiones: Estos datos demuestran que los accidentes ofídicos son frecuentes y constituyen un problema de salud pública en la provincia de Esmeraldas. Se encontró baja mortalidad y complicaciones probablemente por la oportuna administración de suero antiofídico. La mayor frecuencia en hombres agricultores y viviendo en zonas rurales sugiere a las autoridades de salud provincial y nacional la implementación de medidas de protección y educación en estos grupos de riesgo.
Descripción : Introduction: The ophidian accidents are caused by the bite of some species of poisoned snakes. These accidents are emergencies that threat the human life and are prevalent at the tropical forests. At this moment, they are considered as forgotten sickness by the OMS, because of the lack of knowledge or evidences about its epidemiology and risk factors. The treatment with the antivenon substance or its complications, depend of the mild, moderate and/or severe poisoning. The ophidian accidents in Ecuador are considered by the Minister of Public Health as a public health problem because of its high incidence, morbidity, and mortality both in the Coastal and the Amazon region. The Minister of Public Health reported respectively as national levels 1448, 1297 and 1489 ophidian accidents in 2017, 2018 and 2019. In 2014, it was reported a high incidence of these accidents, in the Esmeraldas Province, with a prevalence of young men in farmer activities. Objective: To demonstrate the epidemiology, risk factors and clinic of the ophidian accidents since the data obtained retrospectively (years: 2017, 2018, and 2019) on the clinical histories at the “Delfina Torres de Concha” Hospital in Esmeraldas – Ecuador. Material and methods: Descriptive study of transverse axis since the retrospective analysis of the clinical histories of patients who were attended by ophidian accidents. There were registered socio-demographic and clinic variables as: age, sex, canton of origin, month of the snake bite, specie of the snake, symptoms and signs, anatomical site of the bite, first health aid and complications. The data recollected in Microsoft Excel sheets were analyzed with JASP y SPSS. The statistical analysis included absolute frequencies, proportions and rates. For exploration of the association among the variables, there were used t student and chi square, considering significant to p menor que 0.05. Results: In total: there were registered 110 cases, 26 (24 percent), 22 (20 percent) and 62 (56 percent) for the years 2017, 2018 and 2019 respectively. There was a death (0,9 percent) during the studying period. The male gender with 76 (69,1 percent) cases. The age groups between 11- 20 years old with 25 (22,7 percent) and from 0-10 years old with 21 (19,percent). The farmers with 34 (31 percent), students with 30 (27,3 percent). Cases from Quinindé Canton: 48 (43,6 percent); coming from the rural areas: 68 (61,8 percent); 13(11,8 pervcent), 12(10,9 percent) and 14 (12,7 percent) cases came during the months of May, July and August respectively. Bothrops atrox, was the aggressive snake specie with 43 (41,7 percent) cases. There were diagnosed in 38 (36,9 percent) with moderate and severe severity. As the first aid received are the Anophidic serums in 43 (37,7 percent) with the administration of 5 to 8 blisters in 28 (27,2 percent) cases. In hospitalization, 87,3 percent received among 4 to 12 blisters. The right foot was affected with 35 (34,3 percent) cases. Among the local clinic manifestations predominated: edema in 91 (41,2 percent) and pain in 84 (38 percent). The frequent systematic manifestations were hypertension in 33 (17,6 percent) and tachycardia in 26 (13,9 percent) cases. Complications as compartment syndrome and necrosis, were presented in 7 (6,2 percent) cases. Conclusions: These data show that the ophidian accidents are frequent and constitute a problem in public health in the Esmeraldas province. There were found low mortality and complications probably by the opportune administrations of anophidian serum. The highest frequency in farmer men living in rural zones suggest to the provincial and national health authorities, the implementation of protection and education measures in this risk groups.
