Diabetes Mellitus is a noncommunicable chronic disease that is part of the epidemiological profile of Ecuador, in this sense, according to the WHO (2018) in the last decades has increased progressively the number of cases and the prevalence of the disease, mainly in the countries of moderate and low income Taking these data to figures, the INEC on November 10, 2017 published an infographic which indicates that approximately 1.3 million people suffer from diabetes, considered the second cause of death in the country, which undoubtedly is affecting the population with ever higher rates altering all sectors of the population. (INEC, 2017) In Ecuador, according to the ENSANUT survey, the figures for diabetes are rising progressively, with a prevalence between 10 to 59 years of 1.7 percent, it is estimated that at the age of 50 years at least one in ten Ecuadorian individuals is diabetic ( PAHO. Ecuador, 2018) Objective: Propose a care plan and the algorithm for nursing care to diabetic patients who come to the IESS Central Dispensary. Methodology: The present research study is qualitative-quantitative type, propositive sample, the research technique is the survey, and it was carried out in a Medical Dispensary of the Ecuadorian Social Security Institute of Quito with the participation of 21 nurses who are performing their work. This research seeks to promote the application of a patient assessment tool adapted to the domains and classes of the NANDA, NOC and NIC taxonomy, which allows the creation of a nursing care process, standardized for the patient with diabetes, Based on the Callista Roy Theory, which is an adaptation model so that nurses can have a frame of reference for a better approach to the patient and through Systems Theory instruct the patient towards self-care. Conclusion: in this health care center, the level of training and years of experience of the staff is high, slightly less than half of the participants refer more than 10 years of experience, with a predominant academic level of graduates, the PAE and there is a relationship between these variables and the level of care, with some deficiencies in the development of care plans due to weaknesses in the integration of the NANDA, NOC and NIC Taxonomy. Algorithms and action plans are useful tools for effective care in chronic diseases.en
